Vehicle arrival (shop-in) and completion (shop-out) dates — counted inclusively for days without the vehicle.

The date of the accident / loss for the claim file.

Whether the vehicle was drivable or not at the time of the accident (yes / no).

Rental invoice
If the claimant already rented a car for part of the downtime, attach the rental invoice. Those days are subtracted from the LOU window (inclusive dates).
